 APPROVAL TO PROCEED WITH CUSTOM DESIGN

Dana’s Design Team  has provided sketches, images or models sufficient for me to provide confirmation to proceed with the finished design(s).  I understand that this item is custom designed especially for me and that my 50% deposit paid or gold credit value in lieu of deposit is nonrefundable and that the balance owing will be paid in full upon completion.

I further understand that if I choose to forfeit my 50% deposit that an additional fee of $10.00 per stone will be charged to have my supplied gemstones removed and returned.

                I give my consent for Dana’s Goldsmithing to use any images relating to the custom designed jewellery for marketing purposes.
